There is no support for an automatically generated Index
Using Equations w/o titles results in incorrectly numbered
equations with titles. Use InformalEquation instead.
InlineEquations don't work in the RTF backend. It's not my fault.
Callout support is somewhat fragile.
Line numbering of linespecific displays is somewhat fragile.
In two-sided mode, with the RTF backend, the appropriate
alternation of inner/outer headers and footers does not work
correctly unless %page-number-restart% is true. This is
caused by a limitation in RTF.
CHAR alignment in tables is not supported
The first element of a LISTITEM is formatted as a plain
paragraph, even if it's something else, like a PROGRAMLISTING.
This appears to be necessary in order to get the bullet or
number to the left of the first item to line-up correctly.
The alternative would be to have a special context-rule for
every element that occurs in a LISTITEM and to have special
first-child processing in that rule.
