The explain_fclose_or_die function is used to \f[I]fflush\fP(3) and
\f[I]fclose\fP(3) the given stream.  If there is an error, it will be
reported using \f[I]explain_fclose\fP(3), and then terminates by
calling \f[CW]exit(EXIT_FAILURE)\fP.
