DOOM/Heretic AWE32/64 support

* More Notes:
- Support for the AWE cards have been integrated into lxmusserver
proper. You don't need to apply any patches!
- You NEED linux kernel version >= 2.1.64 preferably 2.2.x to get
AWE support. In fact, I have only tested with 2.2.x. If you
are still using the 2.1 tree then you probably know what you're
doing and can debug it by yourself ;-) Let me know of any bugs
you find though.
- Remember to load a sound font file into the AWE card using sfxload!
This should happen automatically when the driver module loads
(on a properly configured system). But, if you are not getting
any music, yet everything seems to be running fine, check this
out first!
- This should go without saying, but compiling under Linux 2.2.x
and running under 2.0.x will NOT get you AWE support. Things
will compile fine, but the binary will silently fail.
- If you happen to be stuck with non-AWE binary, on a Linux kernel
with AWE support, try using MIDI emulation. I.e.
$ musserv -mu 1

* Notes
This is a patch to DOOM musserver to support AWE32 sound driver.
With this patch, you can play back background music on your AWE32
synthesizer in Linux or FreeBSD.
Since the current patch supports AWE driver v0.3 features,
no sfx conversion is necessary now.
* New Option
-a for specifying AWE32 device.
Musserver searches the synth device and use AWE32 device as default,
so this option is usually unnecessary.
