-- Fixed bug in apply_foreign_keys() that was causing it to fail if there were multiple FKs to the same foreign column. Also fixed a bug that allow the child name parameter to not be passed and didn't handle it if it wasn't. Thanks to Andrew Dunstan for the bug fix and tremendously easier to manage code (Github Issue #64).
