* Created new create routine and removed old create and setup
routines.
* Added ResetIO function call to reset everything per bus
initialization request.
* Found a root cause later but it already was fixed (see
previous log entries below). A bug was in rq_Work function
that cause rq_Work to overwrote pktWork from pktQueue. That's
why a packet got lost during heavy disk load. The rq_Work
already was replaced by new rq_Queue.

* Implemented the initial queue service to process MSCP
commands, flush response packets, etc. by using timer
service. No doubts. NetBSD/vax will be happy with that.
* Blindly fixed a bug that cause a lost (null) packet during
data transfers without my knowledge! That bug leads OpenVMS
to crash after a null/lost packet happened. Root cause
still was unknown because a bug mysteriously disappeared.
* Changed default timers for queue and xfer processes.

* Fixed bugs that cause Ultrix to crash when it attempts
to lock BBR (bad-block replacement) work. By fixing
a bug, force all unit flags have UF_RPL bit.

* Fixed some bugs for compiling without debug facility.
* Finished write access when attempted to install NetBSD/vax
system into MSCP disk drives.
* Implemented command queues with delay timer because of
NetBSD/vax crash because of instanteous I/O access.
Interrupt triggered before 'tsleep' was called. Thank
to Kevin Hardy for extensive debugging.
